Practice Nurse Program

Introduction
The main aim of the Practice Nurse Program is to incorporate Practice Nurses (PNs) as key team players with GPs in a "whole of practice" approach within General Practices.
Fremantle GP Network promotes the importance of PNs in the General Practice Team.
The Practice Nurse Program has two major components:
- Support, mentoring and access to education for current Practice Nurses; and
- Assistance for GPs and Practices to recruit and orientate new Practice Nurses
Fremantle GP Network Support
GP members and Practice staff have access to telephone, email or face-to-face visits for any Practice Nurse queries.
Areas in which FGPN provides PN support include:
- Employing a Practice Nurse
- Accreditation and re-accreditation
- Immunisation
- Enhanced Primary Care Initiative (EPC)
- Practice Incentive Program (PIP)
- Chronic Disease Management
Assistance is always provided through:
- Group or one on one orientation sessions
- PN networking sessions held quarterly to provide peer support and education (combined with Practice Managers and staff)
- Regular education events
