FREO StreetDoctor is a mobile street-based health service for homeless & at-risk marginalised people that provides a visible, easily accessible & non-judgmental healthcare service.  , providing for the physical, mental and social needs of clients in the Fremantle area.

 

Please Note: Freo StreetDoctor does not carry, nor will they prescribe Narcotics or Benzos. Freo Street Doctor does not supply Fit Pacs or needle exchange.

 

Consulting Hours (no appointment necessary):

On average the GP/Registered Nurse consultations last a length of 20-30 minutes each. Patients will be seen in the order they present at the Freo StreetDoctor.  Please let the driver or Outreach worker know if your reason for visiting the van is urgent.

You can telephone the Freo StreetDoctor Coordinator to ask when a particular doctor is next working. Call FGPN office during working hours (9319 0555) or phone the FSD Coordinator at any time (041 9966 087). 

 

Services Available include:

Immunisation, family planning, ECG, pregnancy tests, pap smears, minor surgery, health assessments, care plans, counselling, wound dressing, sexual health, and nutritional advice.

 

Billing Arrangements:

All consultations and procedures are bulk billed.

 

Patient referrals, results and follow-up:

Patients are referred to other services as required; the doctor on duty will advise the follow-up procedure.

 

Reminder System:

Freo StreetDoctor is committed to preventative care.  The duty doctor will seek your permission for you to be included on our reminder system.  We may issue you with a reminder notice from time to time offering you preventative health services appropriate to your care.  If you do not wish to be part of this system please advise the clinical staff.

 

Management of your personal health information:

Your medical record is a confidential document.  It is the policy of Freo StreetDoctor to maintain security of personal health information at all times and to ensure that this information is only available to authorised members of staff.

 

Your rights:

If you have a problem or complaint please feel free to talk to the clinical staff or contact the Coordinator, about it.  We take your concerns, suggestions and complaints seriously. You can also leave Compliments and Complaints in the Suggestion Box.

Weekdays: 8.30am – 4.30pm (excluding public holidays)

Fremantle GP After Hours Practice

Block A Fremantle Hospital, South Terrace, Fremantle.

Weekdays: 7pm – 10pm

Weekends & Public Holidays: 1pm – 9pm

 

With your permission, the doctors at these surgeries can contact Freo StreetDoctor for information about previous care and they can advise us of your visit to them.
