Fremantle Hospital and Health Service (FHHS) GP Liaison works closely with GPs, Divisions of General Practice and staff at FHHS to facilitate the active involvement of GPs in the development of hospital policy and the planning of services which interface with primary care. 

Our primary aim is to:

• Optimise the continuity of care of patients as they move between primary and tertiary sectors

This is achieved through:

• Promoting and improving collaboration, integration and communication betwen GPs and FHHS

 Some key activities include:

      ·        assisting GPs and practice staff to access appropriate care for their patients;

·        assisting GPs and Hospital to improve communication flow relating to patient care;

·        providing up-to-date information re services at the hospital via the FHHS GP Handbook and Internet site;

·        assisting GPs to provide feedback to the hospital re patient care;

·        representing GPs and providing strategic input through membership to key hospital committees;

·        educating hospital staff about the role of General Practice and GPs in the patient journey and provision of resources to assist in communication;

·        periodic surveys of GPs to identify areas requiring attention and to formulate local and statewide recommendations to address issues;

·        development of collaborative initiatives with Divisions of General Practice eg Diabetes Symposium, Discharge Medication Card and the promotion of HMRs;

·        review and update of the Memorandum of Understanding between Fremantle Mental Health Services and Fremantle GP Network; and

·        assisting the hospital to engage with community stakeholders eg Residential Aged Care Facilities regarding patient care, including medication issues.
